We offer a variety of personalized care packages that are tailored to adults of all ages with specialist needs; these include Learning Disabilities, Physical Disabilities, the Autistic Spectrum Disorders, Residential Care, Respite, Dementia, Early onset Dementia, End of Life Care, Acquired BrainInjuries, Neurological Impairment, Mental Health needs and Supported Living within communities.

Residential Care Service

We appreciate that it is a difficult decision for a service user or their families when considering a care home placement. In addition, they are faced with the challenges of going through the process of making the right choice to suit their individual needs and preferences.

Trust Care offer Residential Care for service users with age related challenges such as Early Onset Dementia, Dementia, Physical Difficulties, Learning Difficulties and those with Brain Injuries. We provide packages of care that are tailored to meet the needs of each individual.

Trust Care is committed to making every effort to ensure that this process is as stress free as possible by providing service users and their families with comprehensive information about the service to assist them in making informed decisions. We endeavour to facilitate the provision of a service which encompasses a home away from home experience.

Mental Health Services

Trust Care provides a range of community based mental health care services within the supported living model and residential care for adults with mental health needs which focus on rehabilitative treatment programmes and working within the National Service Framework for Mental Health.

Our focus is on rehabilitation and providing care and support to individuals which enable them to live independently with dignity whilst respecting the individual’s need to exercise choice and ultimately empowering them to reach the highest potential their condition permits through therapeutic interventions, self-medication, social integration strategies and contextualised rehabilitation.

Autistic Spectrum

People with autism say they see the world differently – to them it is a mass of people, places and events which they struggle to make sense of and which can cause them considerable anxiety.

Trust Care develops person centred support packages of care within supported living communities and residential settings which are individualised, needs led and choice driven. This is achieved through developing a conducive environment to live in which meets their needs, consistency in care provision and designing and implementing a community led programme which embraces all opportunities including but not limited to recreational, educational and vocational activities.

Learning Disabilities Core Service

Our residential learning disability services provide accommodation, care and support for people with moderate to severe learning disabilities who may also present with challenging behaviours, and mental health issues. Some residents may be in need of a higher degree of support than available in a standard residential service; our aim is to ensure those residents still have the opportunity for appropriate community involvement.

Each resident is assigned a key-worker to encourage, support and guide them in expressing their own choices, and in setting and accomplishing personal goals to develop the skills they need to live as independently as possible.

The residents benefit from 24 hour support, including those integrated into the community in our domiciliary/supported living houses.
